Hours: 9:00 a.m. – 4:00 p.m.

What to Bring to Your Appointment:

• Unsigned passport application form
• Evidence of U.S. citizenship, plus a photocopy
• Proof of identification, plus a photocopy
• Passport photo
• Check or money order payable to the U.S. Department of State
